한국인 머리뼈의 3차원 영상을 이용한 성별판별: 비계측적 연구

Abstract

This study performed the forensic anthropological sex estimation of Koreans in a non-metric way by reconstructing three-dimensional (3D) computed tomography (CT) images of skulls. The skull CT images used in this study were 100 (51 males, 49 females), and all CT images were taken with a slice thickness of 0.75 mm and then reconstructed into 3D images using the MIMICS 23.0 program. Using the reconstructed 3D image, measurements were repeated twice. The sex determination was male if the 4 point to 5 point was relatively more in five landmarks, and female if the points of 1 to 2 were relatively more. Results of the study show that, 88 of the 100 cases matched the actual sex. Among the 12 discrepant cases, ten cases were mismatched with the actual sex even though the estimation and repeated estimation readout of sex-estimating were the same. Two cases, were “unknown,” showing different sexes in the first and repeated estimations. In conclusion, this study indicated that a forensic anthropological analysis from 3D images provided accurate point information on the landmarks of skulls, showing as high an accuracy as the sex estimation method using real bones. The ten cases of sex mismatch, except the two “Unknown” cases, are considered to be errors that did not consider differences in population groups. In further studies, further establishing a non-metric, specifically Korean methods to increase the accuracy and reliability of sex estimation is need.

서 론

현재 영상의학은 인체의 구조물에 대해 비침습적인 방식으로 환자의 상태나 예후관찰 등 많은 부분에서 활용되고 있을 뿐만 아니라, post-mortem computed tomography (PMCT) 영상은 법의부검 전 사후 시신의 해부학적 상태 및 법의인류 학적 손상분석 등에 사용되고 있다[1,2].
최근 고고학과 법의인류학 분야에서는 많은 사례에서 computed tomography (CT) 영상을 이용한 연구문헌들이 발표되고 있으며, CT 영상을 이용한 연구는 법과학 분야에서 일상적으로 사용하게 되었다[3]. 이는 영상을 통한 간접계측이 파손된 실제 뼈의 계측이 어렵고 온전한 형태의 뼈를 연구에 사용하기 힘들다는 단점을 보완하고, 비침습적 방법이기 때문에 연조직이 존재하는 신체에서도 자료를 확보할 수 있는 점, 연구자료의 저장 장소와 저장 기간 등의 편의성, CT 촬영의 횟수 증가에 따른 다량의 연구자료 확보, three-dimensional (3D) 영상으로 재구성한 경우 축을 따라 쉽게 이미지를 돌려볼 수 있는 등의 장점이 있기 때문이다[4,5]. 법의인류학 감정에서 성별의 감정은 뼈의 표지점을 육안으로 확인하는 대표적인 사례이며. 특히 머리뼈는 남자의 머리뼈가 여자의 머리뼈보다 더 크고 뼈 표지점들이 좀 더 두드러지게 나타나는 등 형태학적 특성을 가장 잘 나타내며 환경적인 요인에 영향을 적게 받아 성별을 판별하는데 유용하게 이용된다[6-8].
고품질과 고해상도는 정확한 3D 재구성 영상의 기본 요소이며, 법의인류학 연구는 3D 영상이 얼마나 세밀한 해부학적 정보를 전해주는가에 따라 그 결과의 정확도가 달라지는데, 영상의 정확도는 기본적으로 사용되는 스케닝 및 모델링의 매개변수와 관련이 있다[3,9,10].
현재 한국인 머리뼈의 3D 영상을 토대로 한 계측방식의 법의인류학 연구는 지속적으로 이루어지고 있으나[11,12], 많은 연구들이 계측방식의 연구이며 비계측 방법을 이용한 성별 판별을 시도한 연구는 거의 없는 실정이기에, 본 연구에서는 한국인의 머리뼈 CT 영상을 3D 영상으로 재구성하여 비계측 방법으로 성별을 판별하였다.

재료 및 방법

국립과학수사연구원 서울과학수사연구소에 법의부검을 의뢰한 한국인 시신의 CT 영상 중 머리뼈의 파손 또는 기형적 이상이 없는 성인 머리뼈 100건을 대상으로 하였다. 100건의 머리뼈 영상은 남성 51건, 여성 49건씩으로 구성되었으며, 사용한 영상의 평균 연령은 남자 38.9세, 여자 39.2세였고, 연구에 사용된 시신의 개인정보 및 성별비율은 감정인에게 비공개로 진행되었다. 본 연구는 국립과학수사연구원 생명윤리위원회에서 승인(2021-05-HR)되었고, 사전동의(informed consent) 면제대상으로 인정되어 사전동의는 요구되지 않았다.
연구에 사용된 CT 장비는 법의학 감정을 위하여 사용 중인 PMCT 장비(SOMATOM AS+, Simens Healthcare, Erlangen, Germany)로서, 보어사이즈 78 cm, field of view 65 cm, scan range 200 cm이다. 촬영조건으로는 관전압 120 kVp, 210 mAs, slice thickness 0.75 mm, pitch factor 0.35, increment 0.7 mm, rotation time 0.3초로 실험을 진행하였고, scan time은 시신의 키에 따라 60-75초이었으며, 촬영방법은 whole body scan으로 진행하였다. PMCT 검사로 얻어진 DICOM 파일은 MIMICS 23.0 (Materialise's Interactive Medical Image Control System, Materialise NV, Leuven, Bilgium) 프로그램을 이용하여 226-3071 Hounsfield units (HU)의 threshold value로 머리뼈의 Image를 추출하고, Computer Aided Design (CAD)로 변환하였다(Fig. 1).

3차원으로 변환된 온머리뼈 영상의 성별 판별을 위해 Buikstra와 Ubelaker [13]의 비계측 성별 판별 방법을 적용하여 다섯 부위의 표지점을 좌·우·위·아래로 영상을 돌려가며 관찰한 후 성별을 판별하였다. 판별은 2회에 걸쳐 반복적으로 진행되었으며, 1차 판별과 2차 판별 사이의 기간은 약 3개월이었다. 각 표지점들의 튀어나온 정도에 따라 1에서 5점으로 분류하였고, 표지점들이 약하게 튀어나오거나 부드럽게 생겼으면 1점에 가까운 점수를 부여하였고, 표지점들이 두드러지게 튀어나오거나 거칠게 생겼으면 5점에 가까운 점수를 부여하였다(Fig. 2). 다섯 표지점의 점수 중 4-5점의 점수가 상대적으로 많을 경우 남자, 1-2점의 점수가 상대적으로 많을 경우 여성으로 판별하였다. 또한, 1차 판별과 2차 판별의 성별이 불일치한 경우 성별모름(unknown)으로 판별하였다. 본 연구의 통계처리는 SPSS ver. 25 (IBM Corp., Armonk, NY, USA)를 사용하여 성별과 각 표지점들 점수들의 연관성을 교차분석하였고, 성별에 따른 1차 판별과 2차 판별의 연관성을 분석하였다.

결 과

본 연구에서는 3D로 변환시킨 머리뼈 영상을 돌려가며 비계측적 방식으로 2회 반복하여 판별하였다. 실제 성별과 감정 성별의 불일치는 10건(10%)이었으며, 1차 성별 과 2차 성별의 판별이 다른 성별 모름이 2건으로, 총 100건의 감정 중 88건(88%)이 실제 성별과 판별 성별이 일치하였다. 10건(10%)의 불일치 중 실제 성별 여성이 남성으로 판별된 경우는 8건(8%), 실제 성별 남성이 여성으로 판별된 경우는 2건(2%)였다. 성별 모름의 경우, 2건 모두 1차 판별에서는 남성, 2차 판별에서는 여성으로 판별되었다.

성별에 따른 1차 판별과 2차 판별을 비교하였을 경우에는, 남성의 경우 다섯 부위 표지점에서 모두 3-5점이 많이 나오는 등 비슷한 경향을 보였으나, 미간(glabella)의 융기와 꼭지돌기(mastoid process)는 5점에서 4점으로의 점수변화가 있었다(Table 3), 여성의 경우는 다섯 부위의 표지점에서 모두 1-3점이 많이 나와 1차와 2차 점수가 비슷한 경향을 보였으나, 미간의 융기에서는 2점이 1점으로, 꼭지돌기에서는 3점이 1점과 2점으로, 목덜미 능선(nuchal crest)는 3점과 4점이 1점과 2점으로 점수변화가 있었다(Table 4). 1차 판별과 2차 판별간 항목별 약간의 점수변화는 있었지만, 2건(2%)을 제외한 98건(98%)의 성별판별이 일치하는 결과를 보였다(Table 4).

고 찰

본 연구는 한국인의 머리뼈를 대상으로 0.75 mm의 슬라이스 두께로 CT 영상을 촬영한 후, 3D 영상으로 재구성하여 비계측적 방법을 적용하여 성별을 판별하였다.
머리뼈와 골반뼈는 성별 판별에 있어 높은 정확도를 가지고 있으며 머리뼈만 사용하였을 경우는 80%-92%, 골반뼈만 사용하였을 경우는 90%-95%, 두 개의 뼈를 사용하였을 경우 98%, 전신의 모든 뼈를 사용하였을 경우 100%의 정확도를 보이며, 인구집단간의 특징을 잘 반영하고 있는 뼈들 중 가장 넓게 이용되고 있는 것이 머리뼈이다[14-17]. 그러나, 머리뼈의 경우 발굴현장이나 범죄현장에서는 부분적 혹은 전체적으로 파손된 상태로 발견되는 경우도 많으며[11], 실제 뼈를 이용하여 표지점을 육안으로 확인하거나 직접 손으로 만져서 판별하는 경우에는 추가 손상이 생길 가능성이 높아진다.
최근 컴퓨터의 발전으로 인해 법의인류학분야에서도 모양 (형태)의 계측 분석이 수행되었고, 각각의 뼈를 대상으로 성별 판별을 위한 회귀분석을 사용하는 시도가 이루어졌으며[18-21], 머리뼈 역시 3D로 재구성한 영상을 이용한 계측방식의 성별 판별 연구가 활발히 이루어지고 있다[22-26]. 3D 영상자료를 토대로 한 법의인류학적 감정의 경우, 판독과 측정의 기준이 되는 신체의 표지점(land mark)들이 매우 정확하게 표현되어야 하며, 이는 영상의 품질과 매우 밀접한 연관성이 있다. 촬영된 CT 영상의 슬라이드의 두께와 조사야(field of view)는 영상의 품질에 큰 영향을 미친다. 일반적으로 해부학적 CT 영상 슬라이드 두께는 0.625-5 mm를 사용하고 있으며, 조사야는 촬영하는 표본의 넓이에 따라 변화하게 된다[27,28].
법의인류학적 감정에 활용되는 영상의 경우 사후 시신의 촬영이기 때문에 방사선노출에 대한 부담감이 적어 더욱 얇은 두께로 슬라이드 수를 늘려 보다 깨끗하고 정확한 영상을 얻을 수 있다는 장점이 있다. Ford와 Decker [27]는 해부학적 특징이나 얇고 세밀한 부분을 3D 재구성을 할 경우, 1.25 mm 이하의 슬라이드 두께를 이용하여야 하며 그 이상의 두께에서는 정확한 해부학적 정보를 얻을 수 없다고 하였다.
Robinson 등[29]은 다리부위를 1.25 mm의 두께로 CT 촬영하여 3D로 재구성한 뼈의 계측 값과 실제 뼈의 계측값의 차이가 없음을 증명하여 CT 영상을 활용한 뼈 계측 연구의 유용성을 주장하였고, Grieser 등[30]은 머리혈관을 대상으로 한 연구에서 0.625 mm의 영상 두께가 1.25 mm에 비해 더 많은 혈관 단위를 제공하여 보다 정확한 혈관정보를 얻을 수 있다고 보고하였다. 또한, Gulhan 등[31]은 살아있는 사람들의 넙다리뼈를 5 mm의 두꺼운 두께로 촬영한 영상을 이용하여 계측방식의 성별추정에 대한 연구를 진행하였고, 성별판별의 정확도가 91%라고 보고하였다. 이에 본 연구에서는 0.75 mm의 슬라이드 두께로 CT 영상을 촬영하여 연구에 활용하였고, 법의인류학에서 측정의 신뢰성과 반복성은 매우 중요한 요소이기 때문에[32], 숙련된 감정인이 반복판별을 하여 신뢰성을 높였다.
본 연구에서 한국인 머리뼈의 3D 영상을 이용하여 비계측적 방법을 이용하여 감정한 결과, 88%의 정확도로 성별을 판별 할 수 있음을 확인하였으며, 이는 0.75 mm의 두께로 촬영된 CT 영상을 토대로 만든 3D 영상이 비계측 연구에서 매우 정확한 뼈 표지점들의 정보를 전달하여 유용하게 사용될 수 있다는 것을 증명하였다.
하지만, Buikstra와 Ubelaker [13]의 비계측 성별추정 방법은 외국의 인구집단을 대상으로 개발된 감정기법으로 한국인을 대상으로 감정을 하였을 때, 인구집단간의 차이로 인해 부정확한 결과를 유발할 수 있다. Kim 등[11]은 한국인 머리뼈 3D 영상을 이용한 성별 판별의 교차유효값은 76.5%-86.2%이며, 한국인 머리뼈와 다른 인구집단의 머리뼈를 계측방식으로 비교하였을 때, 남성는 유럽인보다 머리가 작았고, 여성은 일본인, 인도인보다 머리뼈가 크다고 하였다[10]. 본 연구에서도 10건(10%)의 성별 불일치 중 8건(8%)이 실제성별 여성이 남성으로, 2건(2%)은 실제성별 남성이 여성으로 판별된 경우로, 이는 인구집단의 차이에 의한 감정오류로 추정해 볼 수 있다.
결론적으로, 0.75 mm의 두께로 촬영한 CT 영상을 3D로 재구성하였을 때 머리뼈의 표지점들의 정확한 정보를 제공하여 법의인류학적 감정을 할 수 있으나, 외국인과 한국인의 인구집단 차이를 고려하지 않은 성별 판별 방식으로 인해 한국인의 성별 판별에 오류를 제공할 수 있다고 사료되며, 차후에는 한국인이라는 인구집단에 특성화된 비계측 성별추정 방식이 필요하며, 이는 한국인의 성별 판별 감정의 정확도와 신뢰도를 높일 수 있을 것이라 생각한다.
